Psychological and Neuropsychological Testing Services

Creekview Assessment Center provides comprehensive evaluations for neurodevelopmental, learning, and mental health. Our evaluations use test data to better understand processing strengths and challenges. Creekview psychologists work hard to identify an accurate diagnosis whenever possible. We provide individualized recommendations that help you understand yourself better, meet your future goals, and find effective treatment. Most services are available in Spanish and English.

All assessment types are available for individuals aged 3 years to adult.

Neurodevelopmental Testing

 

This is our specialty area. Neurodevelopmental testing involves a combination of ability testing (which may include cognitive, academic, and processing tests that identify areas of strength and difficulty)  along with a thorough assessment of behavior and mental health symptoms. We know that each person is unique and take that into account when designing your evaluation.

Functional Disability Assessment

 

Functional disability evaluations determine a person's capacity for functioning in particular environment or situation. These evaluations may be required by government agencies as part of your application, or by job sites to evaluate fitness for a particular job or position.

Intake Appointment:    $225.00

​

Our evaluations are individualized, meaning that we do not give the same tests to everyone. Having a detailed intake appointment before determining the total cost of your evaluation ensures we only recommend the tests you really need.

Testing: Price varies by type and complexity

Learning Disability: $1,500 - $4,000

The cost of your feedback appointment and written report summary are included in total cost of your testing evaluation.

OUR PROCEDURES

 

New Patient Inquiry

Complete a new patient inquiry through our website. A psychologist will review your information, match you with an evaluator who can help you, and contact you within 5 business days to schedule an intake appointment. You will need to complete paperwork on the patient portal before your intake appointment takes place.

Intake Appointment

Our first step is to schedule a 60 minute intake appointment, either by telehealth or in-person. During this appointment, a clinician will gather a detailed history and ask you questions. They will make a recommendation about what types of testing you need based on this information. Your clinician will also answer any questions that you may have about testing and schedule your next appointment. There is no obligation to continue with the rest of the evaluation process following intake.

 

Cost Estimate

You will receive a written estimate of how much your evaluation is likely to cost through our patient portal, within 3 days following your intake appointment. Your clinician will recommend necessary tests based on the information you shared during the intake appointment and calculate the cost accordingly.

Testing Sessions

Testing sessions are typically scheduled within 2 to 6 weeks of the initial intake appointment. Testing can take anywhere from 2 to 8 hours, or more, depending on the type of evaluation you need. There will also be components for you to compete on your own at home. Many assessments will require at least one in-person office visit, however, some assessments may be able to be administered virtually by Telehealth.

 

Feedback Appointment

After testing is complete, a 60 minute feedback appointment (telehealth or in-person) is scheduled. Your clinician will share the results of your testing with you, explain any diagnoses, review their recommendations, and take time to answer your questions. 

 

Report Summary

Your clinician will provide a written report summary within a few days of the feedback appointment that includes all of your test data, DSM-5 diagnoses, and a personalized treatment plan and recommendations. 


Superbill/ Receipt

If you have out of network benefits with your medical insurance company, your clinician can provide a superbill/receipt of services for you. These can be submitted to your insurance company for partial reimbursement at the discretion of your insurance company.
